GABA agonists
The role of g-amino-butyric acid (GABA) agonists in treatment of EOR was first suggested by Lidums et al.[Lidums I, 2000] The GABA-b agonist baclofen significantly increased LOS pressure, reduced the rate of transient LOS relaxations, and reduced the rate of reflux episodes compared with placebo in healthy subjects.
There may be some evidence that it is not actually the acid, but other components of the reflux, such as pepsin and bile, which may cause the damage.[Klaus A, 2001] This explains the percentage of patients who remain refractory even after aggressive treatment with PPIs, and may indicate that reduction in gastric acid is not the ideal treatment option for EOR.
